Please be aware that this form expires at midnight on Monday, November 10th. After that time this form will not take registrations and will produce an error.

Meal and seating counts are provided to the caterer no later than noon, Tuesday, Novemer 11th.

If you need to register after the November 10th deadline, please email administration@oceanstatepmi.org, including your Name, OSPMI member status, Company Name and Title (for name badge purposes). We regret we cannot guarantee a meal after the 11/10 deadline.

Note: Please register even if you are attending only for the presentation - it helps us to determine the size of the room we'll need.

CANCELLATION POLICY:

The Ocean State Chapter is bound by contract to pay for each meal reserved for our members. If you reserve a meal and do not attend the event, you will be billed for the missed event. We realize that personal emergencies can arise at the last minute and we are sympathetic to such problems. If you cannot attend an event that you registered for, we request one day's notice to allow us to remove your name from the meal count. Emails regarding cancellations should be sent to administration@oceanstatepmi.org
